This toolkit was part of Susan Waisbren’s , PhD, vision for the Transition to Adult Care Education Initiative for adolescents and young adults with metabolic disorders. The idea originated from Robin Schwartz, MA, genetic counselor, during a session of the New England Consortium of Metabolic Programs. Under Dr. Waisbren’s supervision, Afroditi Veloudaki, MA, compiled, adapted or wrote the contents of this toolkit. As a graduate student of Health Communication at Emerson College she worked with the Children’s Hospital team to design and create the materials that compose this effort. Her supervisor at Emerson College was Timothy Edgar, PhD.
